The Highway Toll Discount for Disability in Japan

If you drive in Japan and have a disability handbook (shougaisha techou / 障害者手帳), you can get a 50% discount on highway tolls. Japan’s expressways (kousoku douro / 高速道路) use distance-based tolling, which means a single trip can easily cost several thousand yen. Over time, a 50% discount adds up to real savings, especially for families who drive regularly for medical appointments, school, therapy, or day programs.

This is one of those benefits that many families in Japan do not know about or assume is complicated to set up. It is actually straightforward, and you do not even need an ETC card to get started. This article walks through who is eligible, how to apply, and how to use the discount at the toll booth.

Who Is Eligible

The toll road disability discount (yuuryou douro shougaisha waribiki / 有料道路障害者割引) is available to holders of the shintai shougaisha techou (身体障害者手帳, physical disability handbook) and the ryouiku techou (療育手帳, rehabilitation disability handbook). There are two driving categories:

Self-driving (honnin unten / 本人運転): The person with the disability is the driver. This applies to holders of the shintai shougaisha techou (身体障害者手帳) Type 1 and Type 2.
Caretaker driving (kaigo unten / 介護運転): A caregiver drives and the person with the disability is a passenger. This applies to holders of the shintai shougaisha techou (身体障害者手帳) Type 1 and holders of the ryouiku techou (療育手帳) with a grade of Ⓐ (maru A) or A.

In other words: if you have a child with an intellectual disability who holds a ryouiku techou (療育手帳) (or ai no techou / 愛の手帳 in Tokyo) with a grade of Ⓐ or A, and you drive them as a passenger, you qualify for the caretaker driving discount. Your child does not need to drive. You do not need your own disability handbook. The discount is based on your child’s handbook.

The seishin shougaisha hoken fukushi techou (精神障害者保健福祉手帳, mental health and welfare handbook) is not currently eligible for the toll road discount. This is different from the train discount system, which was expanded to include this handbook in April 2025. The toll road system has not been updated in the same way as of this writing.

Two Ways to Use the Discount: With or Without ETC

You do not need an ETC card to get the toll road disability discount. This is one of the most common misconceptions about this benefit. There are two routes:

Without ETC: You apply at your ward office, get a sticker on your disability handbook, and use it at staffed toll lanes by showing the handbook to the attendant. No ETC card, no on-board unit (shasai ki / 車載器), no waiting period. You can use the discount the same day you apply.
With ETC: You register your ETC card and vehicle at the ward office, then complete online registration through the expressway discount website. The discount is applied automatically when you pass through ETC gates. This takes about 2 to 3 weeks to activate after online registration.

This article focuses on the without-ETC route first, since it is faster and simpler. If you want to set up ETC later, the process is explained at the end.

Step-by-Step: Applying Without ETC

Step 1: Gather Your Documents

You only need one thing: the disability handbook (shougaisha techou / 障害者手帳) of the person with the disability. That is it. No ETC card, no vehicle paperwork, no driver’s license.

Optionally, if you want to register a specific car at the same time, you can bring the vehicle inspection certificate (jidousha kensashou / 自動車検査証), the driver’s license of the primary driver (unten menkyoshou / 運転免許証), and a family registry document (juuminhyou / 住民票) if the car is registered to a different family member. But registering a car is not required to use the discount.

Step 2: Go to Your Ward Office Welfare Desk

Go to the welfare desk (fukushi tantou madoguchi / 福祉担当窓口) at the ward office (kuyakusho / 区役所) or city office (shiyakusho / 市役所) where the disability handbook is registered. Tell them you want to apply for the toll road disability discount. You can say: “yuuryou douro no shougaisha waribiki wo shinsei shitai” / 有料道路の障害者割引を申請したい (I want to apply for the toll road disability discount).

Fill out the application form (yuuryou douro shougaisha waribiki shinseisho / 有料道路障害者割引申請書). They have blank forms at the desk. If you are not registering a specific car, tell them you are applying without vehicle registration (jidousha touroku nashi / 自動車登録なし).

If approved, they will put a sticker (shoumei shiiru / 証明シール) on the disability handbook right there on the spot. For the self-driving category, the sticker says “Road” (douro / 道路) in red text. For the caretaker driving category, it says “Road Care” (douro kaigo / 道路介護) in red text. The sticker also shows the discount expiration date (waribiki yuukou kigen / 割引有効期限).

The person with the disability does not need to be present. A family member or support person can go as a proxy (dairinin / 代理人).

Step 3: Use It Right Away

Once the sticker is on the handbook, there is no waiting period. You can use the discount on your way home from the ward office if you want.

At the toll booth (ryoukinjo / 料金所):

Drive into a staffed lane. Look for the green sign that says “General” (ippan / 一般) or a mixed lane (ETC/ippan / ETC/一般). Do not go through an ETC-only lane (ETC senyou / ETC専用).
Show the disability handbook with the sticker to the attendant (kakaiin / 係員).
The attendant checks three things: that the person with the disability is in the car, that it is a private vehicle (jikayousha / 自家用車), and that the discount has not expired.
You pay half price (50% off) with cash, credit card, or however you normally pay.

If there is no attendant and only a payment machine (ryoukin seisanki / 料金精算機), press the call button (yobidashi rebaa / 呼出レバー) to summon an attendant. You cannot get the discount through the machine alone.

Important Details to Know

Rental cars and borrowed cars work too. You can use the discount in any private vehicle, not just a registered one. If you are in a rental car (rentakaa / レンタカー), a friend’s car, or a loaner car during vehicle inspection (daisha / 代車), just go through a staffed lane and show the handbook. No extra paperwork is needed.

ETC-only toll gates cannot be used without ETC. Some expressway interchanges are ETC-only (ETC senyou ryoukinjo / ETC専用料金所) and do not have staffed lanes. Smart interchanges (sumaato IC / スマートIC) also require ETC. You will not be able to enter or exit at these without an ETC setup. These are becoming more common, which is worth keeping in mind.

Validity periods. For a new application, the discount is valid until your second birthday from the date of application. For renewals, it is valid until your third birthday from the renewal date. Keep track of the expiration date on the sticker and renew before it expires.

Mirairo ID works. You can show the disability handbook app Mirairo ID (ミライロID) at the toll booth instead of the physical handbook, as long as it is linked through My Number Portal (maina pootaru / マイナポータル). But keep the physical handbook in the car as backup.

Online application is only for ETC. The online application portal at expressway-discount.jp is only available for people who are registering a vehicle and ETC card. If you are not using ETC, you must apply in person at the ward office.

Setting Up With ETC (Optional)

If you want the discount to be applied automatically when you pass through ETC gates, you need to register your ETC card and vehicle. Here is what you need:

The disability handbook
An ETC card in the name of the person with the disability (for caretaker driving, this means your child needs their own ETC card, which can be obtained through a family card linked to your credit card)
The vehicle’s registration document (jidousha kensashou / 自動車検査証)
The on-board unit setup handbook (shasai ki setup shousho / 車載器セットアップ申込書・証明書)

First, go to the ward office and complete the in-person registration (same process as above, but with vehicle and ETC information). Then, complete the online registration at the expressway discount portal. It takes about 2 to 3 weeks for the ETC discount to become active after online registration.

Once active, you can pass through ETC gates automatically and the 50% discount is applied to your bill. This is more convenient for regular highway users, but the in-person route works perfectly fine for occasional trips.

Practical Tips

Start without ETC. If you are unsure whether it is worth setting up ETC, start with the sticker-only method. You can always add ETC registration later. The sticker works immediately and costs nothing.
Keep the handbook in the car. If you are using the discount regularly, keep the handbook in the car so you always have it when you need it. For families with a child who has a disability, this means keeping the child’s handbook in the car rather than at home.
Plan your route around staffed lanes. If you are not using ETC, check in advance whether the interchanges you plan to use have staffed lanes. Some newer interchanges are ETC-only.
Do not forget the passenger. For caretaker driving, the person with the disability must be in the car. The attendant will check. If you are driving alone without the handbook holder in the car, the discount does not apply.
Renew before expiration. The discount does not automatically renew. Go back to the ward office before your sticker expires to avoid any gap in coverage.
